Student Music Therapist (SMT) Practica Agreement
University of Dayton Student Music Therapists (SMT) enrolled in practica courses are responsible for knowing and complying with the following guidelines:
1) The SMT will be aware of and abide by the American Music Therapy Association (AMTA) Standards of Clinical Practice and Code of Ethics in all aspects of their practica experiences.
2) The SMT will abide by all facility rules of conduct in an effort to aid the facility in achieving its stated mission.
3) The SMT’s first priority is the clinical site and their clients. As such, events or activities of a personal or academic nature should never interfere with or supersede the commitment to the clinical work. The SMT shall contact the academic and on-site supervisors as soon as possible regarding emergencies of a medical or personal nature that inhibit the SMT's ability to be on-site and perform their duties.
4) The SMT will notify the academic supervisor regarding clinical problems or concerns that interfere with client treatment or the SMT's ability to comply with these guidelines.
5) The SMT will follow immediately all directives of the on-site supervisor or other staff appointed by the on-site supervisor in the event of and emergency or at the professional discretion of the on-site supervisor.
6) The SMT will strive to create and maintain positive professional relationships with all facility staff.
7) The SMT will perform academic tasks required by the academic and on-site supervisors such as reading or writing about professional literature germane to the population served or toward fulfilling the role as a health care professional.
8) The SMT will complete all documentation required by academic and on-site supervisors in a thorough, timely, and accurate manner.
9) The SMT will maintain confidentiality regarding all patient interactions and documentation. The SMT is free to share such information with the academic supervisor in the context of the supervisory relationship and in practica labs.
10) The SMT will adhere to an established practicum schedule as coordinated with the academic or on-site supervisors. The SMT will endeavor to accommodate scheduling changes initiated by the on-site supervisor.
11) The SMT will be fully prepared both musically and clinically.
12) The SMT will provide or arrange for all equipment necessary for carrying out required duties.
13) The SMT will dress appropriately for the professional environment.
14) The SMT will be punctual for all appointments at the clinical site.
I have read and understand the above guidelines. I understand it is my responsibility to follow these guidelines to the best of my ability. I also understand that the grade I receive for my practicum course may be affected by non-compliance with these guidelines.
