COURSE EVALUATION FORM

 

Course evaluations are extremely important for assessing the quality of instruction in a course. These evaluations give feedback to the instructor, reinforcing positive aspects of the course and allowing him/her the opportunity to make modifications in future offerings of the course in light of students' perceptions. Evaluations also provide information to the Chair of the department offering the course regarding the quality of the courses that the department sponsors.

The evaluation for on-line courses has been designed specifically for the on-line environment. Click on the link below to access the evaluation for this course.

You will need to enter your Lotus Notes internet username and password (the same ones you used for accessing Lotus Notes from the internet) to gain access to the evaluation. QuickPlace now uses your LDAP username and password. The LDAP username may be the same as the username for Lotus Notes. However, the password for LDAP may be different.

Remember that the instructor will not see the results of this evaluation until 30 days after the final grade has been submitted. THIS EVALUATION MUST BE COMPLETED WITHIN ONE WEEK OF THE END OF THE COURSE.
