Ohio Admin. Code § 3701-3-03 Reportable Disease
Notification
(A) The physician in attendance of a case or suspect case of a
disease which is required by law to be reported, including all
"class A", "class B", and "class C"
categories of disease designated as reportable under rule 3701-3- 02 of
the Administrative Code, shall submit a case report to the board of
health of the health district in which the patient resides.
(B) A person in charge of a hospital, clinic, or other institution
providing care or treatment, having knowledge of a reportable disease
case or suspect case, shall submit a case report to the board of health
unless there is evidence that the case has already been reported.
(C) Any individual having knowledge of a person suffering from a
disease suspected of being communicable is authorized to report to the
board of health all known facts relating to the case or incident.
